INTRODUCTION
Impotence is a widespread problem that affects many millions of men. It can occur at any age and at any point in a man's sexual life. The myth of impotence as an old man's disease has been , shattered. Impotence is a problem of men but also affects couples and families. Now as a result of recent medical advances, impotence need no longer cause frustration, embarrassment and tension. New solutions are now available for an age-old problem. Impotence no longer has to be, a barrier to a sexually active and satisfying relationship. Everyday more and more men are discovering that they too can overcome impotence.
WHAT IS IMPOTENCE
A mall who is impotent is persistently unable to get or keep an erection that is
firm enough for sexual intercourse. He usually still has the ability to have an
orgasm and father a child but has difficulty in doing these things because he just can not get an erection. Impotence is not normal and is by no means an inevitable part of aging.
HOW DOES AN ERECTION OCCUR
An erection is an involuntary reaction. Sexual stimulation and excitement cause the brain, nerves, heart, blood vessels and hormones to work together to produce a rapid increase in the amount of blood flowing into the penis. The blood becomes trapped and held in the two spongy chambers in the shaft of the penis. As the chambers
fill rapidly with blood, they expand and the penis becomes firm and elongated. The result in an erection.
WHAT CAUSES IMPOTENCE
A man's inability to get or maintain an erection can be caused by physical or psychological causes. Many men who suffer from impotence are still told that their problem is all in the mind. But we, at the Alpha One Andrology Group Know better. With the sophisticated tests available at our disposal, it has been proved that in 90% of patients, a cause for impotence can be demonstrated and therefore, treated. Once the problem has been
identified, proper treatment is given to help them return to a satisfying sex life.
PSYCHOLOGICAL IMPOTENCEPsychological impotence is impotence caused by a man's anxiety or fear that he may not be able to perform well
sexually. This is often referred to as 'performance anxiety' or 'fear of
failure'. A man who is worried about getting an erection cannot relax and probably will not get an erection. Persistent psychological failure can be caused by too much drinking, fatigue, anger, stress, worry, guilt or any number of emotional factors.
PHYSICAL IMPOTENCE
Physical Impotence is impotence caused secondary to a physical disorder of the body. Injuries, disease) hormonal. imbalances and operations can all lead to physical impotence. Diseases include diabetes, high blood pressure, and surgery on the prostate, bladder and lower GI tract. Alcohol, tobacco, drugs and certain medicines can also interfere with erections and cause physical impotence.
DETERMINING THE. CAUSE
A battery of sophisticated investigations are performed under the' direct supervision of director and consultants of Alpha One Centre. Over a series of interviews with the patient, the cause is isolated and the treatment started.
TREATMENT
Psychological impotence is treated by counseling or sex therapy with over a number of sessions with a psychiatrist.
In the treatment of physical impotence, once the cause is isolated and treated, primary treatment of impotence is commenced. Various forms of treatment are now available to treat erectile problems.
VACUUM ERECTION DEVICE
Some form of impotence can be treated by a Vacuum Erection Device. This works by placing a cylinder over the penis and activating a pump mechanism. The vacuum draws blood into the penis, creating an
erection. A rubber' ring is place around the base of the penis keeping the erection
firm. The. ring cannot be used beyond 45 minutes at one time and the patient has to be instructed in the use off the device by a
qualified Doctor.
SELF INJECTION THERAPY
Another treatment option is to use penile self-injection therapy. A medicine is injected directly into the penis to produce an erection in just 3
minutes. The erection lasts for 30-45 minutes and the injection is almost painless. After the first demonstration by Dr. Dhir, patients can continue with self-injection at their own homes.
ORAL MEDICATION
Drug like sildenafil can be taken orally to manage erectile dysfunction. Taken an hour before sexual activity they work by enhancing the
effects of nitric Oxide, a chemical that relaxes smooth muscles in the penis during sexual stimulation and allows increased blood flow. While oral medicines improve the response. to sexual stimulation, they do not trigger an automatic erection as injections do. These drugs and other forms of treatment should be used only under physicians supervision.
SURGERY
In the most stubborn cases that do not respond to any of the less invasive measures, a penile implant is inserted into the penis as a permanent solution to the impotence problem. Two types of penile implants are available :the inflatable implants and the flexible rod implants. Both enable the impotent man to have a satisfactory erection for satisfactory intercourse. The flexible rods produce .a
permanently.
Firm penis and the inflatable rods allow a controlled erection. Both forms of surgery are performed at the Alpha One
Centre and the patient is usually goes home on the second day of the operation.
HOW DO I PROCEED
Impotence is not an inevitable part of aging and it is not a cross that a man has to bear forever. It is treatable and virtually every man can be helped.
